ShayLee Cares, Inc., will not refuse admission to any individual on the grounds of race, sex, national origin, or any other non-merit factor, in accordance with all state and federal regulation. We will not refuse admission on the grounds of age, except where funded programs eligibility criteria indicate age restrictions.
ShayLee Cares, Inc., Social Worker will assess the needs, desires, and resources available for the prospective recipient and link them to the services and resources required for that individual. A Plan of Care will be develop to outline the needs, goals, objectives, strengths, weakness, needs and wants of the recipient. It is our goal to assist the recipient with all areas of his/her daily living needs and to ensure that he/her function as normal of a life as possible within the home and community. The recipient, as well as the family and/or support system, will participates in the assessment phase as well as the selection of the Direct Support Professional (DSP) who will be working with the individual.
In order to qualify for supports services a person must have a developmental disability as defined by the Developmental Disability Law in Louisiana Revised Statute 28:451.2.
A Developmental Disability is a severe, chronic disability that is attributable to an intellectual or physical impairment or compination of intellectual and physicall impairments.
And/Or
Is manifested before the person reaches age twenty-two; is likely to continue indefinitely; results in substantial functional limitations in three or more of the following areas of major life activity: 1. self-care 2. receptive and expressive language 3. learning 4. mobility 5. self-direction 6. capacity for independent living or economic self-sufficiency 7. is not attributed solely to mental illness 8. reflects the person's need for a combination and sequence of special, interdisciplinary, or generic care, treatment, or other services which are of lifelong or extended duration and are individually planned and coordinated, or 9. a substantial developmental delay or specific congenital or acquired condition in a person from birth through age nine which, without services and support has a high probability of resulting in those criteria (listed above) later in life that may be considered to be a developmental disability.
